Separate changes in evidence from changes in language

Record new observations, revised targets, status changes and amended agreements separately. A repeated target is not a new achieved milestone. More confident wording is not an additional production record. If management offers a cause for a change, retain it as an attributed explanation and seek the supporting operational context.
